New Patient Forms
If you would like to save time and avoid the distraction of juggling clipboards, cell phones, and insurance cards, we provide our patients the option to complete their new patient forms from the comfort of their homes before they arrive.
Affordable Financing Available
Many patients avoid dental care because they are concerned that they won’t be able to afford treatment. We accept payments in cash, check and major credit cards, and if patients are unable to fully cover their out of pocket costs for any reason, we work closely with CareCredit, third-party healthcare, and dental financier, to offer patients low or 0% interest financing.
